Periodontally Accelerated Orthodontic Therapy Course

The South African Society for Periodontology is proud to present Dr. Colin Richman, a Wits graduate, living in Atlanta. I was highly impressed by lectures on PAOO at the AAP meeting in Denver last year. Colin mentioned that he has experience in this technique and is prepared to share some of his experience with us during his visit here. Colin emphasized that “the more general dentists you can get to the PAOO meeting the better. They are the ‘gatekeepers’ of the patient, and they need to ‘Demand’ these procedures from the orthodontists and periodontists.”

Herewith a description of the contents of the course by Dr Colin Richman

Periodontally Accelerated Orthodontic Therapy “PAOO” (formerly known as Wilckodontics).

Patients with malocclusion, mutilated occlusion, or patients in need of Comprehensive Dental Rehabilitation frequently require Orthodontic Treatment in order to obtain a predictable, aesthetic and functional treatment outcome. Many of these patients have existing moderate to advanced Periodontitis and Gingival Recession as well. They therefore also require Periodontal and corrective Muco-gingival treatment to treat the existing periodontal conditions. However, these patients perceive the orthodontic treatment time of 2-3 years as a strong hindrance to orthodontic treatment and thus ideal reconstructive treatment.

By combining the needed periodontal surgical care, together with PAOO, plus Orthodontic treatment, patients and restorative dentists now have a new modality of treatment that patients are more willing to accept. But this technique is also indicated for patients without existing periodontal conditions.

PAOO will also significantly decrease or eliminate the following potential complications of adult Orthodontic Treatment:

  • Gingival Recession

  • Apical Root Resorption

  • Relapse

  • Time required for comprehensive orthodontic therapy can be reduced by 60-70 %.

  • Enhanced facial profiles.

 

Basically, PAOO has at least five major positive facets, all of which are documented in the Orthodontic literature.

These include:

  • Comprehensive Orthodontic treatment in approximately 6-9 months.

  • Less gingival recession, and where recession is present, adjunctive and prophylactic treatment of the recession sites prior to implementing Orthodontic Treatment

  • Greater Orthodontic stability post treatment

  • Less apical resorption

  • Predictable and definitive periodontal and muco-gingival treatment, where indicated.

 

Additionally, Skeletal Anchorage is becoming a recognized adjunct in Orthodontic treatment. I recently attended a program by Junji Sugawara, (Chairman, Department of Orthodontics, Tohoku University, Japan) on the Skeletal Anchorage System, and his results are amazing! SAS would be a valid modality to intrude mandibular molars for correction of open bite, effectively intruded maxillary molars and move mandibular molars for distally correcting anterior crossbites, malocclusions characterized by mandibular anterior crowding, and dental asymmetries.

The program I have put together will address the following topics:

  1. Biological principles and concepts of the PAOO treatment technique.

  2. Treated and completed cases demonstrating different aspects of PAOO

  3. Patient testimonials and literature support of PAOO.

  4. Concepts of Skeletal Anchorage Systems.

  5. Pathogenesis, prevention and treatment of Gingival Recession.
